- Have not signed up with any other Christmas program AND
- Have children in the household under the age of 16 (you can only sign up your own children)

Please complete the following form and submit ONLY 1 FORM. This is the first step in the application, someone will follow up with you within 2 weeks (sooner if possible) with next steps. Note: Keep in mind that this is an income eligible program and that an application doesn’t guarantee adoption. We will try our best to ensure every child is adopted.
